What defines psychosis?

Psychosis is characterized by a total loss of connection with reality. Individuals experiencing psychosis may have distorted perceptions of reality, which can manifest as delusions (false beliefs) or hallucinations (seeing or hearing things that are not present). This condition significantly impacts one's ability to think clearly, communicate effectively, and respond appropriately to their environment.

In contrast, the other choices describe states that are not indicative of psychosis. A strong connection with reality, clear and coherent thought processes, and a mild form of anxiety indicate functioning within the bounds of reality and logical reasoning. These states are associated with mental health conditions where reality testing remains intact, unlike psychosis, where such testing is profoundly compromised.
